Our Recovery Specialists Provide Comprehensive Case Management

The case management specialists at The GateHouse craft individualized aftercare services that are vital to long-term sobriety.

Upon entering our residential extended care program, each patient is assigned a case manager who will meet with him/her at least three times during their stay. The first of these important meetings will occur within seven days of admission, followed by another consult midway through residential treatment, and another one week prior to completion of their program. We also encourage our clients to seek out additional meetings if and when necessary.

Re-entering the Workplace

While receiving the full benefits of residential treatment at The GateHouse, clients work diligently with our case managers to plan and coordinate support services that can assist with re-entering the workplace. These services include:

  • GED preparation
  • Networking assistance
  • Résumé guidance
  • Interview coaching

The case manager also assists with acquiring personal identification, as well as accessing other available local resources, including primary care physicians, eye doctors, and dentists.

Aftercare

Even as clients complete their residential program at The GateHouse, communication with case managers continues in an effort to monitor and sustain individual progress. Meetings occur one week, one month, and three months after discharge. During these check-ins, a follow-up questionnaire is completed jointly to determine any areas where additional support may be warranted.
