For Treatment Providers Referring to Our Residential Treatment Centers

In an effort to streamline our referral process and continue to work on improving relationships, we would like to ask that the following documents be sent via fax to our Centralized Intake Department:

  • Client’s Signed Consent
  • Client’s completed biopsychosocial with assessment
  • Client’s history and physical with vitals signed by a physician or CRNP
  • Client’s Psychiatrist Evaluation (if available)
  • Client’s most recent medication record
  • Client’s tentative discharge date
  • Clients legal history, open warrants, and upcoming court dates.


In addition to the above items we ask that you and the client review the following documents:
